OCS APPLICATION PROCESS

This Step by Step process will explain to you the OCS Checklist and what you need in order to have a complete packet. You will also be able to find the pdf documents or any other document required to complete the application. Good luck!

 

Step 1.) OCS Student Enrollement Prerequisite Checklist- This check list makes up the entire OCS packet. When completed the entire packet must be submited to your region 2IC.

Step 2.) OCS Emergency Contact Information Sheet- Also known as the Student Data Information Sheet.

Step 3.) OCS Application- Complete this form and sign on the bottom left. Unit commander will also sign page 1 only. Page 2 requires Board Endorsement signatures from 3 officers: President of Board should be (0-4 or higher) and 2 Board members should be (0-1 or higher). This includes all enlisted soldiers who are MOS of 09S.

Step 4.) Letter of Recommendation- Letter should be from an 0-3 at minimum. Failure to submit results in non-enrollment to OCS. All enlisted soldiers even MOS of 09S must include a letter of recommendation.

Step 5.) Copy of Social Security Card- Also accepted is a verification letter from Social Security Administration showing SSN.

Step 6.) PQR or DA 4836- ETS date must be later than end of course.

Step 7.) Birth Certificate- Age at commissioning should not exceed 41 years and 364 days. Include a legible copy (DO NOT USE ORIGINAL) of birth certificate (from the Bureau of Vital Statistics) in the OCS packet. A verification of Birth Form used by recruiters to enlist individual will not be acceptable.

Step 8.) Proof of Citizenship- (If applicable) If you are a naturalized citizen you must bring your original certificate of naturalization in order for OCS to issue a MEMO of Naturalization (DO NOT COPY CERTIFICATE).
